Are you a diabetic struggling to constantly monitor your insulin levels, eat the right foods, stay active or feel healthy and happy?



Inspiro's diabetes education and assessments services offer education and advice to support people managing diabetes. These services are available at our Lilydale and Belgrave sites.


Inspiro’s Diabetes Management Program is committed to providing high quality, comprehensive diabetes education to people with diabetes, their families and carers health care professionals.
 

Not taking control of diabetes can cause more health problems


Diabetes can be a debilitating and frustrating disease, potentially causing severe foot problems and a range of other issues including oral health, teeth and mouth problems.. Managing your insulin levels, exercising and eating the right foods can be hard. It is important to keep your blood sugar levels in check, otherwise other health problems may develop.


After waiting longer than she should have, my mother took the first step and saw a diabetes educator. She learned how to take control of her life, feel better, eliminate highs and lows in her blood sugar levels and take action to live a healthier life.

What you can expect at your first visit to Inspiro


We recognise that taking the first step can be challenging and you will be treated with respect and care throughout the consultation. Following assessment your diabetes management programme is personalized to your needs.


Diabetes Assessment and Referral Service


This is a free service for people with diabetes. It includes an initial assessment and referral to Inspiro's Dietitian, Podiatrist and Diabetes Nurse Educator as required.


These health professional can help you manage:
  • Diabetes, heart disease and stroke risk
  • Healthy eating and weight control
  • Physical activity
  • Stress management and improving sleep patterns
  • Goal setting and managing lapses
  • Get support and motivation to achieve and
  • Maintain healthy lifestyle goals

About Inspiro's Diabetes Education Program


The Inspiro’s Diabetes Education Program has four distinct classes that have each been tailored to help people with diabetes and their family members better understand the condition. To get the best out of what we offer and to comprehensively learn all of the best practices surrounding diabetes, take all four classes.


The classes are:
  • Understanding Diabetes Course
  • Healthy Living Support Program
  • Health & Vitality: Groups and programs
  • Healthy Supermarket Tours

Course 1: Understanding Diabetes Course


Do you have type 2 diabetes? Get support to manage your condition, make lifestyle, changes and improve your health! Our program is designed to support you to manage your condition with confidence.


This is a four week course for people with type 2 diabetes. This course is $7 per session. The course includes four sessions run on a Thursday.


Your program will include:
 

  • One on one support from an Inspiro health worker
  • An initial assessment and goal setting session to develop a health plan
  • Reviews at 3 months, 6 months and 12 months
  • Referrals to other health services and programs
  • Ongoing communication with your GP

 

Course 2: Healthy Living Support Program


A 12-month program where you are assigned a dedicated health worker to help you set and reach your realistic health goals. Our health worker will help you to manage your diabetes to get the best results.

Course 3: Healthy Supermarket Tours


As part of the two-hour tour, a Diabetes Australia dietitian will show you how to read and understand food labels and select the items that are healthy, delicious and helpful for managing your diabetes.


A comprehensive pocket size healthy shopping guide booklet is included as part of the tour cost and helps to take all the hard work out of food shopping.

Course 4: Nutrition & Healthy Eating Workshops


Nutrition and Healthy Eating workshops are held monthly and run by an Accredited Practising Dietitian. They teach a great easy-to-follow model to maintain healthy eating habits, help you take control of your eating and improve your relationship with food.


Dates: 3rd Tuesday of the month 5.00 – 6.30pm.
Location:  17 Clarke Street, Lilydale.
Cost: $20 per session, 3 sessions for $50.
