Inspiro diabetes clinic - Integrated Diabetes Education and Assessment Service (IDEAS)

On Wednesday 27th February, the Integrated Diabetes Education and Assessment Service (IDEAS) clinic was launched at Inspiro. This clinic focuses on supporting clients to self-manage their Type 2 Diabetes and will be delivered by Inspiro from our Lilydale site, located at 17 Clarke Street Lilydale.

High rates of preventable hospital admissions for type 2 diabetes in the Yarra Ranges


With the Yarra Ranges having one of the highest rates of preventable hospital admissions for type 2 diabetes in Melbourne, the (IDEAS) clinic aims to meet immediate community needs.


1.7 million people in Australia with diabetes

There are currently 1.7 million people in Australia with diabetes, but it's not just those individuals, it's their families that are impacted as well; diabetes impacts a huge number of people in our community. The aim of the clinic is to support clients to self-manage their Type 2 Diabetes, helping to prevent some of the complications that come from diabetes like blindness, kidney disease and other issues that can arise if people with diabetes are not looked after well.

The clinic is the culmination of a number of years of effort. It is a multidisciplinary team approach that is person centred. The specialised care team focus aims to prevent people’s symptoms worsening and requiring hospitalisation by providing effective management and treatment.


Endocrinologists, credentialed diabetes nurse educators, nurses and allied health practitioners will assist clients

The clinic provides care in a community setting and includes an Endocrinologist, credentialed diabetes nurse educators, nurses and allied health practitioners. The clinic aims to provide services in one place, at the one time, diverting care out of the hospital setting and reducing demand in our local hospitals whilst providing convenience to clients.


Forming partnerships between community health, local hospitals and general practice

Inspiro Community Health Service aims to improve access and the health of people living in the Yarra Ranges. Strong partnerships between community health, local hospitals and general practice mean that we can provide effective diabetes care, based in people’s communities, delivered in a way that means they are actively engaged to achieve the health outcomes they need.


60 people enrolled already!

Many people referred to by their GP for Type 2 Diabetes to Eastern Health Specialist Diabetes Clinics are now having their diabetes managed closer to home at an IDEAS clinic, like the Inspiro IDEAS clinic in Lilydale. With 60 clients enrolled already, and many more to come, the service is proving to be an important and much needed community resource.

This clinic is supported by Australian Government funding through the Eastern Melbourne Primary Care Network, and the service is delivered by Inspiro Community Health for residents of the Yarra Ranges.

Location, time and date

The service is based at Inspiro: Community Health Service, 17 Clarke St, Lilydale, and operates on a Wednesday afternoon.


Inspiro provides a range of diabetes assistance classes and programs

Other Diabetes services provided by Inspiro include exercise classes for people with type 2 diabetes run by a university qualified exercise physiologist,  a four week course for people with type 2 diabetes that helps participants to learn how to manage their condition with confidence, 

Additional Inspiro diabetes services include the Healthy Living Support Program, a 12-month program where you are assigned a dedicated health worker to help you set and reach your health goals and link you to other health services, a diabetes assessment service; a free service for people with diabetes that includes an initial assessment and referral to an Inspiro's Dietitian, Podiatrist and Diabetes Nurse Educator as required.

The Life! Program (not for people diagnosed with diabetes)

Being active, maintaining a healthy weight and eating healthy are just some of the important lifestyle changes you can make to reduce your risk of diabetes, heart disease and stroke. Inspiro run a free diabetes Australia VIC program includes one individual and five group sessions with a health professional.
